二、申请人的资格要求：1.满足《中华人民共和国政府釆购法》第二十二条规定，提供下列材料：

1）供应商必须是具有独立承担民事责任能力的在中华人民共和国境内注册的法人或其他组织，投标时提交有效的营业执照（或事业单位法人登记证等相关证明）复印件；

2）供应商必须具有良好的商业信誉和健全的财务会计制度，投标时提交2019年或2020年财务状况报告复印件(新成立公司提供成立至今的月或季度财务报表复印件)或银行出具的资信证明材料复印件；

3）具备履行合同所必需的设备和专业技术能力，投标时提交资格声明函；

4）有依法缴纳税收和社会保障资金的良好记录，投标时提交2021年任意一个月的依法缴纳税收和社会保险的证明（如缴费凭证）复印件，如依法免税的或不需要缴纳社会保障资金的，应提交相应文件证明其依法免税或不需要缴纳社会保障资金；

合同包1(从化区公路管养中心2021年国省道小修保养维护服务)特定资格要求如下:

（1）供应商须具备交通主管部门核发的公路养护工程从业单位三类甲级资质及有效的安全生产许可证；

(2)投标人须提供《公平竞争承诺书》原件，格式详见3-3《公平竞争承诺书》；

(3)单位负责人为同一人或者存在直接控股、管理关系的不同供应商，不得参加同一包组投标或者未划分包组的同一招标项目的政府采购活动（提供资格声明函）。如同时参加，则评审时均作无效投标处理；

(4)为采购项目提供整体设计、规范编制或者项目管理、监理、检测等服务的供应商，不得再参加该采购项目的其他采购活动（提供资格声明函）；

(5)本项目不接受联合体投标；

(6)本项目只接受已购买本项目采购文件的供应商提交的投标文件。
