二、申请人的资格要求：

1.投标供应商应具备《政府采购法》第二十二条规定的条件，提供下列材料：

1）具有独立承担民事责任的能力：提供有效期内的工商营业执照(或事业单位法人证书，或社会团体法人登记证书)、组织机构代码证及税务登记证复印件（或“三证合一”营业执照复印件）。

2）有依法缴纳税收和社会保障资金的良好记录：1、提供2021年07月份或之后任意一个月的依法缴纳税收的证明文件复印件。无需缴纳税收的供应商，则须提供相关部门出具的免税证明文件。【缴纳税收的证明文件是指：加盖银行业务章的税收回单，或加盖税务部门电子征税专用章的税收电子转账专用完税证，或加盖税务部门公章的纳税证明；如供应商在规定的时间段内没有发生业务的，则提供税务部门出具的纳税证明，或加盖税务部门公章的纳税申报表】。2、提供社会保险基金管理部门（或税务部门）出具的单位参加社会保险的证明文件原件，或从社会保险基金管理部门（或税务部门）网站的单位参加社会保险参保证明（证明内容须包含有效期内的查验办法和专用章等）。无需参加社会保险的供应商，须提供相关部门出具的证明文件。【参保时间须为2021年07月份或之后任意一个月。】

4）履行合同所必须的设备和专业技术能力：1、投标人须具有市政公用工程施工总承包叁级或以上资质。2、投标人须具有测绘行政主管部门颁发的测绘乙级或以上资质（专业范围须包括地下管线测量）。3、本项目不接受分公司投标，接受联合体投标。若为联合体投标，联合体的成员数量不得超过2个，且必须以市政公用工程施工总承包叁级或以上资质为主体单位，联合投标单位需在投标文件中提供《联合体共同投标协议书》。

5）参加采购活动前3年内，在经营活动中没有重大违法记录：须提供：1、政府采购供应商反商业贿赂承诺书。2、三年内无重大违法记录承诺书。
